To remove problems in settlement of social insurance and health insurance benefits for each foreign worker due to differences in surname and name on his/her social insurance book, health insurance card with those on his/her passports, visas, etc. Ho Chi Minh City Social Insurance Agency requests that with regard to cases of new increase registration from April 20th, 2023, on declaration forms (Form TK1-TS, Form D02-LT, Form D03-TS), the enterprise shall declare surname and name of the foreign worker in line with the order on the passport.

With regard to the cases where foreigners are participating in social insurance, health insurance, social insurance agencies shall send the List of participants at enterprises for reviewing information on passports/visas/temporary residence cards. If surname and name of any foreigner are recorded by the social insurance agency not conform to the aforesaid principle (in line with the order on his/her passport), the enterprise shall make dossier 608 and submit it to the social insurance agency for adjustments.

In case the enterprise fails to coordinate in reviewing, adjusting surname and name of the foreign worker or it is late for reviewing, adjusting that affect the settlement of social insurance and health insurance benefits, the enterprise shall take responsibilities.

This Official letter replaces Official letter No. 1639/BHXH-TST dated April 14th, 2023 regarding guidelines on foreigner's individual dossier of participation in social insurance, health insurance.
